Clinic ownership
Approved (example)Trade licence from the City Corporation, in the clinic's name.
Location
Approved (example)A photo of the signboard and entrance, plus a map pin dropped at the door.
Public phone
Approved (example)Confirmed on the clinic's listed number.
DLS registration
Under reviewRegistration certificate from the Department of Livestock Services.
BVC-registered veterinarians
1 matched 1 pending 1 missingFor each vet: full name, BVC registration number and certificate. BVA membership can be added too. It shows on the vet's profile but doesn't count as registration.
Drug licence (DGDA)
Not submittedOnly if you run an in-house pharmacy. Otherwise, mark it as not applicable.
